Long-Term Rehabilitation

JGS Lifecare’s Long-Term Rehabilitation, in partnership with HealthPRO® Rehabilitation, provides expert therapeutic care, offered onsite at the Leavitt Family Jewish Home. Our staff prioritizes every resident’s adjustment to illness or injury and designs services to meet every patient’s individual needs. Patients’ families are also part of every comprehensive care plan, receiving support and counseling to make a smooth adjustment to care.

We admit individuals who may need complex medical/surgical services, IV therapy, comprehensive medical rehabilitation and respiratory therapy, and patients who have been hospitalized with neurological and orthopedic conditions.

All skilled services are provided under the direction of a multi-disciplinary team, including physicians, nurses, physical therapists, occupational therapists, speech-language pathologists, social workers, recreational therapists, consulting specialists, and dietitians, who work closely with the patient and family to develop an individualized plan of care.

Occupational Therapy

Occupational therapy assists patients recuperating from physical illness or injury. Therapy encourages rehabilitation through the performance of activities required in daily life such as self-care, feeding, and leisure activities. OT can help with a wide range of diagnoses and disabilities including:

  • Strokes/TIAS
  • Traumatic brain injury
  • Neurologic disorders
  • Joint replacements
  • Pathological fractures
  • Arthritis
  • Chronic pain
  • Wounds

Physical Therapy:

Physical therapy treats disease, injury, or deformity using physical methods such as massage, heat treatment, and exercise. Physical therapy may be used in conjunction with drug therapy and surgery or in place of it. PT can help with a wide range of diagnoses and disabilities including:

  • Strokes/TIAS
  • Traumatic brain injury
  • Neurologic disorders
  • Joint replacements
  • Pathological fractures
  • Arthritis
  • Chronic pain
  • Wounds
  • Amputations
  • General debility

Speech Language Therapy:

Speech language pathologists work to prevent, assess, diagnose, and treat speech, language, social and cognitive communication, and swallowing disorders in children and adults. SLT can help with a wide range of diagnoses and disabilities including:

  • Strokes/TIAS
  • Traumatic brain injury
  • Neurologic disorders
  • Dysphagia
  • Arthritis
  • Dementia
